Age Limit – 4 TO 5 years as on 31.03.2026 (Date of Birth of the child should be between 1.04.2021 and 31.03.2022)
The Date of Issue of the Birth Certificate should be within 1 year from the Actual Date of Birth of the child.
The following will be required to be uploaded at the time of online submission of the Form:

Self Attested Birth Certificate

Self Attested hospital birth record slip.

Copy of Aadhar card of both the parents (Compulsory) and the applicant child (if available)

Self Attested copy of PAN card of both the parents.

Caste Certificate in case of SC/ST/OBC.

Blood group report of the child.

Passport size Photograph of the Applicant child.

Post card size Family photograph (only Father, Mother and the child).
